Director of Social Work
AristaCare At Cedar Oaks - Director of Social Work (Full Time)AristaCare is seeking a Director of Social Work to join our teamIn this role, you will provide medically-related social services to attain or maintain the highest practicable physical, mental, and psycho-social wellbeing of each patient.
Develop and implement case management services to all residents.
Maintain contact with family (with resident's permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning.
Maintain progress notes under federal and state guidelines for Long Term Care.
Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents' individual needs.
Requirements:
LCSW or CSW is requiredMust be a licensed social worker.
BA from an approved Social Work school is required.
Subacute and geriatric experienceMinimum one years' experience with seniorsIn Supervisory capacity in a hospital, skilled nursing care facility, or other related medical facilities.
